Les inscriptions pour la saison 2022/2023 sont ouvertes!
L'inscription se fait en ligne et le paiement aussi.
Le certificat médical est valable 3 ans, un an après l'avoir obtenu, il doit être complété par une attestation de non-contre indication à la pratique sportive sous forme d'un questionnaire de santé.
1/ Téléchargez maintenant le document QS-Sport, pour à la page suivante répondre aux questions et cochez J'atteste avoir répondu "non" à toutes les questions du questionnaire santé, ou alors J'ai répondu oui à une question et je joins le certificat médical.
2/ Vous devrez aussi télécharger la demande de licence et d'assurance ci-dessous, FEDE licence, la remplir, la signer, puis l'uploader plus loin dans l'inscription. C'est un document qui ira à la FFH.
3/ Vous aurez plusieurs documents à télécharger ci-dessous, à compléter, signer et à uplader à la page suivante.
4/ Cliquez sur "s'inscrire", remplissez toutes les données et uploadez les documents indiqués,
5/ Vous recevrez un mail avec un lien pour le paiement une fois votre demande de réinscription validée par nos soins, vous pourrez payer en plusieurs fois,

Registrations for the 2022/2023 season are open! Payment is now online.
The medical certificate is valid for 3 years, after one year, it must be supplemented by a health questionnaire called QS-sport.
1/ Download the QS-Sport document now, so you will be able to answer the questions on the next page and tick I certify that I answered "no" to all the questions in the health questionnaire, or I answered yes to at least one question and I attach the medical certificate.
2/ You will also need to download the license and insurance application below, FEDE Licence fill it out, sign it, then upload it on next registration page.
This is a document that will go to the French Hockey Fededation.
3/ A few documents have to be downloaded, completed, signed and uploaded on next page.
4/ Click on "s'inscrire", fill in all your personnal data and upload the mandatory documents.
5/ You will receive an email with a link for payment once your registration request has been validated by us, you can pay in several instalments.

En m' inscrivant, je prends l'engagement de me conformer aux statuts (ci-joints), aux décisions du Comité directeur de l'association.
Je prends acte que la cotisation est due pour l'année entière, aucun remboursement ne pourra être pris en compte, que ce soit pour raison médicale ou indisponibilité des installations sportives, indépendantes de la volonté du CFP.
J'ai pris acte de l'assurance souscrite par le CFP auprès de la compagnie MACIF.
